Memorial Bench Order Form

Please fill out the following information to purchase a personalized memorial bench.  Bench orders are placed between April 1st and September 1st each year.  The Parks Department Director will contact you when your bench is ready to be placed.  Please note that due to delays in manufacturing and shipping that orders are now taking up to 6 months to arrive.  

Please mail or drop (at the Town Clerks Office) a $450 check made payable to:
